Passport (MRP) Re-Issue / Rules for Change / Correction of Amendments and Application Form

Filling this form for renewal of passport, photocopy of current passport and receipt of money deposited in the bank should be attached and the original passport must be brought together.
In case of information change / correction, the relevant certificates should be attached.
A) Permanent officers / employees of government, semi-educator, autonomous and state-owned enterprises and husband / wife of government employees, and dependent on government servants, children aged 15 (fifteen) years will receive emergency benefits by depositing a general fee. In this case, submit to the non-certification certificate (NOC) website from the respective organizations.
B) Retired government servants and their spouses will get emergency benefits by submitting a general fee. In this case, the retirement certificate will be submitted.
C) Required documents required for re-issue application for change of existing machine readable passport information.
 
Due to changeable data papers
1. Partial change of name of the name / parent / mother is not suitable for change.
2. The full name change of name / parent / mother is not suitable for change.
3. Nikahnama / Talaknama (where applicable) in case of marital status / divorce.
4. Birth date change is not suitable to change.
5. Permanent Address Police Report (1 copied re-issue form for this and 1 copy attested new application form - DIP Form -1).
      B. Due to the re-issue of the application and the passport issued, the old passport must be brought together.
